ACTIVE MEMBER- SCACM SPRING MEETING 2021 REGISTRATION
Active SCACM Member (2021 dues previously paid) given full access to Symposium lectures, Interest Group sessions, Exhibits and Exhibitor Workshops, and Social Networking Activities plus access to recordings of this educational materials at NO ADDITIONAL CHARGE. Note: Attendance at a SCACM Workshops is an additional fee.
NON-MEMBER- SCACM SPRING MEETING 2021 REGISTRATION – $20.00
Payment of this fee gives registrant an Active SCACM 1 Year Membership and full access to Symposium lectures, Interest Group sessions, Exhibits and Exhibitor Workshops, and Social Networking Activities plus access to recordings of this educational materials. Note: Attendance at a SCACM Workshops is an additional fee.
NON-MEMBER- SCACM SPRING MEETING 2021 REGISTRATION- KSL Sponsored
IMPORTANT: Obtain the Registration Code from KPHL BEFORE you select this option. The KY Public Health Lab (KPHL) is sponsoring attendance for those who live or work in KY. Payment of this fee by KPHL gives registrant an Active SCACM 1 Year Membership and full access to Symposium lectures, Interest Group sessions, Exhibits and Exhibitor Workshops, and Social Networking Activities plus access to recordings of this educational materials. Note: Attendance at a SCACM Workshop can also be included in this sponsorship, but does require an additional entry during this registration to select from option(s) available.

This Meeting registration includes ALL of the following:

- SCACM SYMPOSIUM LECTURES: Four 1-hour sessions
- SCACM INTEREST GROUP (IG) SESSIONS: Three 1-hour sessions
- VENDOR EXHIBITS and WORKSHOPS: Four 1-hour sessions
- POSTER SESSIONS
- NETWORKING OPPORTUNITIES

For a small additional fee you can register to attend a SCACM WORKSHOP.

Preliminary Program (updated 2/20/2021)

Attendees: All SCACM sponsored lectures, IG sessions and workshops provide P.A.C.E.®-approved educational credits to those who complete a roster and who show full attendance via the electronic check-in & out system that will be used for each educational activity. Note that all sessions will be recorded and available for this credit until December 31, 2021. More details provided upon registering.

To Begin your MEETING and/or SCACM WORKSHOP Registration:

MEMBERS: Select the MEMBER registration, and when prompted, login to your Member Portal to qualify for that rate. Contact scacm@scacm.org if you need help with your portal login.
NON-MEMBERS: Select NON-MEMBER registration. Once you pay the NON-MEMBER registration rate, our SCACM office will approve a 1 Year SCACM Membership for the remainder of 2021 for you.
Scroll through and select from the additional list of opportunities the specific workshop &/or networking event you wish to attend.
When prompted, click "Register" to provide us with your requested events and contact information.
If you wish to pay by Purchase Order, Invoice, or Check, select "Invoice Me" at the end of your registration.
Should you need a SCACM W-9, our 2021 W-9 is available HERE.
CANCELLATION POLICY: In the event that you cannot attend the live event(s) for which you registered and do not wish to access any of the recorded meeting/workshop educational materials, the SCACM office MUST be notified prior to March 7. SCACM will NOT issue refunds after March 7.
